To verify if you have completed a biometric screening and/or health assessment to earn an employer contribution of up to $800 to your health savings account (HSA):
- Log in to Your Benefits Center
- Click on View My Personal Info on the left hand side of the page
- Scroll to the bottom of the page
Your employer contribution will be deposited to your HSA and your record on the site will be updated to COMPLETE by the 20th of the next month following your completion of a health assessment or biometric screening.
For example: Your health assessment was completed on January 17. Your employer contribution and record on Your Benefits Center will indicate COMPLETE by February 20th.
The employer contribution for completing a biometric screening is $200 if you have employee only medical coverage and $400 for employee + dependent(s) medical coverages. You will earn an additional $200 or $400 for completion of a health assessment. The contribution is pro-rated for mid-year enrollees.
