Biometric Screening and Health Assessment Q&A

What is a biometric screening?
The screening consists of a blood draw, blood pressure check and measurement of your waist circumference.

How do I complete my screening?
Go to www.yourbenefitscenter.com/screening for information. The screening is free if you have the screening completed at your workplace, use a Quest Diagnostic Center or an in-network doctor/provider. To find a Quest Center near you, go to https://my.questforhealth.com.

Will an onsite screening be held at my workplace?
Onsite biometric screenings are held in late January/early February and September each year in the Charlotte, Irvine, Nashville, New York City, Tulsa and Reston offices.  For a list of the February onsite screening dates/times, click here.

What happens to my screening information?
After your screening, you will receive a report containing important numbers such as your cholesterol and sugar levels–information that can tell if you are at risk of certain conditions such as heart disease.

What is the health risk assessment?
The health assessment is an online questionnaire that helps you learn about your current health status. The questions cover your lifestyle, health history and recent health screenings (such as annual blood work). It takes about 15-20 minutes to complete and when you are done, you will receive a personalized health report.

Will my screening results and the health assessment information be kept private?
Yes. No one at your employer will be able to see your screening results and your health assessment information.

How much can I earn by completing a screening and/or health assessment?
If you are covering just yourself, you will receive $200 for completing each action. If you are covering a spouse, children, or both, you’ll receive $400 for completing each action. Amounts are prorated for mid-year enrollees.

When will I receive the employer contribution?
The employer contribution will be deposited to your HSA generally by the 20th of the following month.  For example, if you complete a biometric screening in January, your employer incentive will be deposited to your HSA generally by the 20th of February.
