We all know that taking care of our health is important. And, that what you don’t know, CAN hurt you.
You can take a step towards recognizing your own special health strengths and weaknesses. Complete the online health assessment and learn what you are doing right, and where there’s room for improvement. Take control of your health. And, earn an annual employer contribution to your Health Savings Account (HSA) while doing so.
- Log on to Aetna Navigator.
If you are not registered, use the “Register” link in the First Time Users box to get started.
- Select the “Complete Your Health Assessment” link on the homepage.
The questionnaire takes approximately 20 minutes to complete and is 100% confidential. Your employer will never see the results.
- View your personalized health report and action plan.
It will provide you with next steps you can take to maintain and/or improve your health.
- Claim your incentive.
You will receive an employer contribution to your HSA by the 20th of the next month following your completion of the assessment. The employer contribution is $200 for employee only medical coverage or $400 for all other medical coverage categories (i.e., employee + spouse, employee + children, family coverage). The employer contribution is prorated for mid-year enrollees.
